LDAP Authentication error: Splunk failed to fetch form values for form 'eaiform'

I am currently testing Splunk and have it setup using LDAP to connect to AD, which appears to be working (is detecting AD Groups and Users which can be used for login) but whenever I click on the 'authentication method' screen I get the following error: Splunk failed to fetch form values for form 'eaiform'

Any idea why were getting this and how we go about resolving the issue?

The message appears to be benign and should not be impacting your use of LDAP. I have confirmed that all of my users are still able to authenticate and I can modify role assignements without issue.

This issue will be resolved in a future maintenance release

I can confirm that I am also running 4.1.5 and this was a new/clean install.
