help on stylesheeet form with script form

How to add a form stylesheet and a form script both please?

I done this but its wrong

<form stylesheet="format.css", script="tokenlinks.js">

Could you help me please??

Please refer https://docs.splunk.com/Documentation/Splunk/8.1.0/AdvancedDev/UseCSS

<form stylesheet="<style_filename>.css" script="<script_filename>.js">

You should place these files in `/appserver/static` folder and refresh splunk web
