Where to find SA-ModularInput-Windows

I want to use Splunk App for Microsoft SQL Server as per the document
http://apps.splunk.com/app/1524/#/documentation. This app comes with TA-SQLServer for the universal forwarder.
Now TA-SQLServer needs the following Add-ons for working:
- Splunk_TA_windows
- SA-ModularInput-Windows
But where can I find SA-ModularInput-Windows?

I dont see SA-ModularInput-Windows in the link you provided... but what you missing is :

The Supporting Add-on for PowerShell (SA-ModularInput-PowerShell).
