TA-connectivity: Why I am getting error "In handler 'ping': not all arguments converted during string formatting" when creating a data input?

I've installed TA-Connectivity app and when trying to create a data input for Ping, I am getting the following error:

In handler 'ping': not all arguments converted during string formatting Encountered the following error while trying to save: In handler 'ping': not all arguments converted during string formatting

And the data input is not created.

I am using Splunk 6.3.2

My configuration is as follows:

name: /SDM/splunk/etc/apps/TA-connectivity/lookups/ping_salsa.txt
host_field: hostname
interval: 600
sourcetype: ping-cust
Host: LT-10.54.3.2
index: ping-cust

The file ping_salsa.txt:

hostname,port,url
10.54.3.2,,

In splunkd.log I see:
