Splunk Db Connect app -- Getting "Login failed for user 40053. ClientConnectionId:662899f0-1eb3-476f-b0d5-5db2494404db"

I'm trying to connect splunk to SQL server but I'm getting login failed .
I've attached my input values . could you please take a look and see how you can help me to fix this issue ?

Splunk Version : 7.0.1
App name : Splunk DB Connect

Since I'm facing same issue. Could you please let me know how was it resolved

Also just wanted to mentioned the user that I putted has been created in sql server as a user and it's already activated .
