Need help with my first chart

Hey Guys,

I have a free TV beside me and I've been told to setup a dashboard for two of our servers. I need to include transactions/second (MSSQL), connections(MSSQL) and Current CPU usage. Can anyone help me get this started? having a hard time finding examples.
