How to configure Splunk DB Connect 2 for MS SQL Server 2012?

I already have SPLUNK installed. On this, I want to use it for generating various reports from an MS SQL Server database.

The reports are as below:
1. Db growth
2. CPU utilization graph/trend
3. IO utilization graph/trend
4. User queries as per their execution time, killing time etc
5. App users details for the above point

Splunk App for Stream and Splunk App for Windows Infrastructure would be a good starting point. The universal forwarder will install the Windows TA (if you select it), which will give you your perfmon counters for CPU and IO, while Stream can monitor for SQL user queries. There's a TA for SQL as well for other health/logging options, and there's DB Connect as well.
