How to set up the XML for form refresh on a dashboard?

I am trying to set up a refresh for a form that was created. I did not create the form. I have created other dashboards having the refresh built into it.

I tried to follow the advice from How to Refresh a simple FORM dashboard every 60 seconds (https://answers.splunk.com/answers/76679/how-to-refresh-a-simple-form-dashboard-every-60-seconds.htm...) but nothing I'm trying is working.

The beginning of the XML is

<form>
  <label>name</label>

An answer from the other question says to put <form refresh="30"> at the top of the page. This hasn't been working for me.

<form><form refresh="30">
  <label>name</label>

Am I missing something or is this not the correct placement for this?

If it matters, I am running 6.1.
