Why am I getting a "page not found" error when trying to configure the Splunk for Cisco Identity Services (ISE) App?

I have installed Splunk Version 6.4.2, Splunk Build 00f5bb3fa822, and Splunk for Cisco Identity Services (ISE) App Version 2.0.4. I cannot configure the app. When I tried to configure the app by clicking 'continue app page setup page', I am getting 'Page not found' error. Is it a bug?
