Please share a SPL to list all Forwarders, hosts they are installed on and how often / time the FWs are "calling home"

Please share to list all Forwarders, host they are installed on and how often / time the FWs are "calling home". I am looking for healthy FWs, when date / time they are set to call home. Is it possible to get a list of Broken / Unhealthy FWs as well? Thank u very much in advance.
