How do I add a form to my dashboard that has a combination of a text and drop-down input for filtering?

I want to add an input form to take an input from text as well as drop-down.

I'm not sure if you want a cascading functionality, where one input affects the values available in the next, but there is a "Cascading Form Input Element" example that might help.

I am looking to add a single input filter which takes user entered text / selected item from dropdown (not cascading these two filters).
