Any apps that allow you to add notes to a View?

Does anyone know of any apps that would allow you to add notes or annotations to a view dynamically? The use case would be a diagnostic view where users might want to add notes or comments about problems that have been previously observed/diagnosed. This might give other users the benefit of past experience...

It might be overkill, but there's the Sideview Editor which is a part of Sideview Utils and which released back in Sideview Utils 2.0. You can use the Editor to edit advanced xml dashboards, and adding in an HTML module and adding a note with it, is not terribly complicated.

Granted, it wont work for simple XML dashboards, so there's that.
