Try to see the link http://localhost:8000/dj/en-us/splunk_wftoolkit/form/ after installing Web Framework Toolkit app. And, follow also the link C:\Program Files\Splunk\etc\apps\splunk_wftoolkit\django\splunk_wftoolkit\templates and edit form.html template.
