Does the Splunk App for Microsoft Exchange needs to be installed on Indexer as well ?

Throughout the installation manual of the app, it says to install the app only on the Search Head.
However, I noticed on the "Troubleshoot the Splunk App for Microsoft Exchange" page, it says, "Confirm that the Splunk App for Microsoft Exchange resides on all indexers and search heads in the deployment."

Does it really needs to be installed on the Indexer ?

The way that it is packaged today, yes. Feel free to deconstruct it into several smaller apps and send the minimal stuff to the right places, but I wouldn't bother.

In clustered environment, the main application needs to be installed on Search Head. Wheres TA (Technology Add-on) will be required on Forwarder. It must be installed on indexer in case you have any index time extraction or you are using universal forwarder. The TA is needed on Search head in case there are any search time extraction.

For Microsoft Exchange App, you don't need to install it on indexer, it's needed only on search head.
