Add-on for New Relic v 2.1.0: Missing Account ID and Insights API Key after upgrading

After performing the upgrade of Splunk Add-on for New Relic from v 2.0.0 to v 2.1.0 the Account ID and Insights API key are missing from the input configuration.

Are there any known issues with this upgrade process?
