How to configure Mysql monitoring extension

I am running machine agent on Linux, installed using rpm. How can i configure the MySQL extension, extension available in GIT is not well documented.

In past I download extension from Appdynamics website but I do not find it any more. extension available in GIT does not seems to me ready to use, No proper instruction give.
