By default customer application-name associated with agent under not system account but user applications are associated under customer1 (default user(non-root user) account) , can you login customer account (if single tenant customer1) and see if no apps exists? Let us know if any disconnect in our understanding of the issue
Provide all apps view in addition to license screen in ui, if issue persists

Actually it happened after seting to "not set" the detail license information for "end user monitoring" in admin.jsp page when the attached error seen on the screen
then by restarting the server in next login i faced to loose of applications.
lest say the main issue is the new attachment error

We confirm the provided error in UI error is already fixed in 4.1.2 release version which is yet to release, Hope that information helps
