Why is the RWI Executive Dashboard set up is blank after pressing Setup Page Button?

Hello,

I have downloaded the RWI Executive Dashboard App, The RWI Executive add-on, and the MS Teams Add-on.  When I go an set up the RWI Executive Dashboard app, I get a blank screen after I press the Contine to app set up page button.

I have Enterprise 8.1, Running on Centos 8

any help would be greatly appreciated.
