I have an error during configuring "Splunk Add-on for Box" Apps on splunk server.

In order to access to the Box API, our Splunk server needs to use proxy. We put proxy configuration on setup screen as below.
Box Proxy Setup
[Checked] Enable Proxy
Proxy host
---.---.--- (Proxy host name)
Proxy port
----- (Proxy portname)
We have tried to install and configure Version 1.0.1. Any help will be greatly appreciated.
