I would start by creating a simple view that runs a search on load that pulls splunk and gets the list of indexes back, I would then use that list to populate a search and have the search when the drop down is changed

see http://docs.splunk.com/Documentation/Splunk/5.0/Viz/Buildandeditforms

for starters

try this URL

http://docs.splunk.com/Documentation/Splunk/4.3.4/Developer/AddDropDowns

I found that most of the stuff breaks that uses latest instead of teh release number in the URL path
