Got any error output from the message displayed or from the web service logs at time of setup?

You could try this search at the time of the incident and then maybe we can figure out what's going on.

index=_internal sourcetype=splunk_web_service OR sourcetype=splunkd

The other thing you can do is just manually call the app set up. The setup page exists largely as a workaround for issues windows search head users will have with lookups and upgraders will have. If you are not on windows and are not upgrading try just going into the app's app.conf

$SPLUNKHOME/etc/apps/splunk_for_vmware/local/app.conf

If the file/directory does not exist create it. You want to put the following in it to work around the issue:

[install]
is_configured = 1

fyi if you are upgrading just disable the DA apps from 1.x versions of the app manually and this workaround will still work perfectly.
